Six Key Factors Influencing Ketamine Dosage


In my practice, I consider six main factors when determining the ideal ketamine dosage for each patient: age, experience with psychedelics, history of trauma, one's ability to "let go," body weight, and known history of sensitivity to medications. The form I use most in practice is intramuscular (IM) and dosing ranges below are given in that context.

Prior exposure to psychedelic substances can impact a person's response to ketamine. Those with previous experience may have a higher tolerance or be more comfortable navigating altered states of consciousness. In such cases, we may consider a slightly higher initial ketamine dosage.


Trauma History: Finding the Right Balance

For individuals with a history of trauma, ketamine therapy can be both powerful and challenging. In these cases, it's essential to start with a lower ketamine dosage to ensure a gentle and supportive therapeutic experience.


One's Ability to "Let Go":

The willingness to surrender and trust the process can significantly impact a patient's response to ketamine therapy. Those who struggle to "let go" may benefit from a more cautious ketamine dosage approach, ensuring a comfortable and secure experience.

A recent research article by Wilkinson et al. (2021) supports the use of a weight-based dosing framework for ketamine therapy. The study suggests a starting dose of 0.5 mg/kg, with a typical dosage range of 40 mg to 110 mg, depending on individual factors and clinical context. This research highlights the importance of personalizing ketamine dosages to optimize therapeutic outcomes and minimize side effects.


Known History of Sensitivity to Medications:

For patients with a known history of sensitivity to medications, careful attention must be paid to ketamine dosage. It may be necessary to start with a lower dose and adjust gradually to prevent adverse reactions while still ensuring therapeutic effectiveness.


The Ketamine Dosage Spectrum: From Relational Work to Ego Dissolution


In general, it is a common practice to start with a low ketamine dosage to assess a patient's unique response to ketamine. This approach allows healthcare providers to carefully monitor the individual's experience, needs, and goals, and adjust the ketamine dosage accordingly. Ketamine dosage is indeed a spectrum, with lower doses proving beneficial for relational and introspective work, while higher doses can induce profound, transformative experiences that some might describe as akin to "ego dissolution."


The higher ketamine dosages, while potentially more challenging, can facilitate a deep reconnection with oneself and a broader perspective on life. The lower ketamine dosages, on the other hand, can provide a gentler, more grounded experience that fosters personal growth and self-awareness.
